Uploading external patient records

Upload PDF records to a patient chart, review file requirements, and manage uploaded documents.

The Records tab allows you to securely store clinical documents from external providers, laboratories, referral sources, or patients. This includes completed worksheets, prior diagnostic assessments, referral letters, medical reports, and other supporting documentation.

Uploading a Record

To upload an external document:

  1. Open the Patient Dashboard.

  2. Select the Records tab.

  3. Click Upload Document.

  4. Select a PDF file from your computer.

  5. Click Done to complete the upload.

A confirmation notification appears after each successful upload.

Upload Requirements

When uploading records, keep the following requirements in mind:

  • Documents must be in PDF format.

  • Individual files can be up to 50 MB.

  • Uploaded records become immediately available to clinicians and other users who have permission to view the patient's records, such as supervisors.

  • Records remain in the patient's profile until they are deleted.

Viewing and Managing Records

Within the Records tab, you can:

  • Open a document by selecting its file name.

  • Search for records using the search bar.

  • Delete a document by selecting the Trash icon.

Deleting a document permanently removes it from the patient's record.

Clinical Best Practices

Maintaining prior evaluations, laboratory results, referral documents, and historical records in the Records tab helps ensure that members of the care team are working from the same clinical information.

For easier searching and organization, use descriptive file names whenever possible. For example:

  • 2023_Neuropsych_Evaluation.pdf

  • Referral_Letter_Dr_Smith.pdf

  • CBC_Lab_Results_March_2025.pdf

What file types can I upload?

Only PDF documents can be uploaded to the Records tab.

Is there a file size limit?

Yes. Each uploaded document can be up to 50 MB.

Who can view uploaded records?

Records are visible to users who have permission to access the patient's records, such as the treating clinician and authorized supervisors.

Can I delete a record after uploading it?

Yes. Select the Trash icon next to the document to permanently remove it.
