How to Send a Form to a Patient

You can provide forms to your patients at two stages: during the initial intake process or at any point during their treatment via the Patient Dashboard.

Automatic Onboarding Forms

When you add a new patient to Allia, any forms you have previously marked as Default in your settings will be automatically selected for the invitation.

  • Customization: You can remove any default forms or add additional ones from your library before sending the invite.

  • The Patient Experience: Once the patient creates their account, these forms appear as a Task List on the home screen of the Allia Client App. These tasks stay prioritized on their home screen until completed, ensuring you get your paperwork back before the first appointment.


Sending Forms from the Patient Dashboard

If you need a patient to sign a new Release of Information (ROI) or update their policies mid-treatment, you can send forms individually.

  1. Open the Patient Dashboard: Go to the specific patient and select the Medical Data tab.

  2. Access Forms: Select the Forms sub-tab and click Send Form.

  3. Select Your Template: A searchable list of your saved templates will appear.

    • Tip: Use the search bar for quick access to specific assessments or state-required forms.

  4. Need to add something new? If the form isn't in your library yet, click Upload to add it on the spot:

    • Upload the PDF (up to 50 MB).

    • Rename it for the patient’s view.

    • ✅ Check if a signature is required.

  5. Confirm: Click Send.


Tracking Completion

Once a form is sent, its status will appear in the patient’s file as Pending.

  • Real-Time Updates: As soon as the patient completes the form in their app, the status will change to Completed.

  • Storage: Completed forms are automatically stored in the patient’s Medical Data section, where you can view or download them at any time.
