How to Send a Form to a Patient

You can provide forms to your patients at two stages: during the initial intake process or at any point during their treatment via the Patient Dashboard.

Sending Forms During Patient Onboarding

When inviting a new patient, any forms configured as Default Forms are automatically selected.

Before sending the invitation, you can:

  • Remove default forms

  • Add additional forms from your library

Once the patient creates their account, the forms appear as tasks in the Allia Patient App/Portal.

These tasks remain visible until completed.


Sending Forms from the Patient Dashboard

  1. Open the patient's record.

  2. Navigate to Medical Data.

  3. Open the Forms tab.

  4. Click Send Form.


Select a Form

You'll see two sections:

My Forms

Forms you've created.

Other Forms

Forms shared by other users within your practice.

You may send these forms if your permissions allow.

Use the search bar to quickly find a template.

Need to add something new? If the form isn't in your library yet, click Upload to add it on the spot:

  • Upload the PDF (up to 50 MB).

  • Rename it for the patient’s view.

  • ✅ Check if a signature is required.

Preview Before Sending

Select a form and click Preview to review it before sending.


Set an Optional Deadline

After selecting a form, click Next.

Choose whether the form should have a deadline.

No

The form is sent without a due date.

Yes

Select a future date from the calendar.

Patients will see this due date when completing the form.


Send the Form

Review your selections and click Send.

The patient immediately receives the task within the Allia Patient App/Portal.


Tracking Completion

After sending a form, its status appears within the patient's Forms section.

Completed forms are automatically stored within the patient's Medical Data for future reference.
