Service Facilities – Managing Your Practice Locations

Service facilities represent the physical or billing locations where services are provided. This information is included in box 32 in the claim form.

Steps to Manage Service Facilities:

  1. Go to Billing → Service Facilities.

  2. You’ll see a table with: Facility Name, Address, City, State, Facility Type, NPI, Actions (Edit/Delete).

  3. To create a new facility:

    • Click "+ Create New Service Facility".

    • Fill in Facility Name, Address, City, ZIP, State, Facility Type, NPI.

    • Optionally, Mark as Default.

    • Click Create Service Facility to save.

  4. Existing facilities can be modified using Edit or removed using Delete.
