Allia supports forms you sign, read-only documents, and forms you fill out.
Access your forms
To access your forms:
Tap My Forms from Home or Profile.
Choose Pending or Completed.
Form types
Sign form: Read and sign the form as required.
View document: Read the document without completing any fields.
Fillable form: Answer multiple-choice, short-answer, or long-answer questions.
Completed forms generate a PDF for you and your clinician.
Note: You can download forms at any time from the Profile tab.
