The patient reviews the document and signs it before submitting when a signature is required.

The patient completes the interactive PDF fields and signs before submitting when required.

Dynamic Forms

The patient answers questions built in Allia and submits the completed form.

Complete a Dynamic Form for a patient

You can complete a Dynamic Form for a patient during an appointment, enter responses collected verbally, or help someone who cannot complete the form independently.

To complete a Dynamic Form for a patient:

  1. Open the patient's Dynamic Form.

  2. Select Fill for Patient.

  3. Enter the responses.

  4. Save your progress to continue later, or submit the completed form.

Important: To prevent conflicting edits, the clinician and patient should not complete the same Dynamic Form at the same time.

Digital signatures

For a form that requires a signature, the patient:

  1. Opens the form.

  2. Reviews or completes the required information.

  3. Signs electronically.

  4. Submits the form.

The form status then changes to Completed.
